What exactly does the phone show?

The plain text you encoded, with no intermediate page. The phone sometimes offers to search the content on the web.

Is there a length limit?

Technically up to ~2,900 characters, but a QR code scans best when it stays short. Under 300 characters is ideal.

Can anyone read the text?

Yes, anyone can scan the code and read it. Don't put genuinely sensitive information in it.

Does it work without internet?

Yes: decoding happens entirely on the phone. It's one of the advantages of pure text over a link.
